FOIA Requests

DHIN is committed to conducting its business in an open and transparent manner, in accordance with the expectations pertaining to a State-related institution.

To access your personal health information as contained in DHIN’s patient record, please enroll in our patient portal, Health Check Connect, here. You may also complete, have notarized and return by mail this form: My DHIN Health Records.

To submit a Freedom of Information Act (FOIA) request, as outlined in Delaware’s Freedom of Information Act, please use the form below. (Requests for personal health information are not covered by FOIA; please follow the instructions above for those requests.)

For FOIA requests, we may need more information regarding your request and will contact you accordingly. Some requests may take up to 10 business days to complete. Please note, requests for copies greater than 20 pages will require a fee of .10 per page greater than 20 pages.
