A New Community Health Record is Here!

Please note that call volumes through DHIN’s Service Desk are higher than usual, and you may experience longer wait times by phone. We encourage you to use the self-service ticket function available in the CHR or submit a ticket through https://dhin.my.site.com/dhincustomerfacing.

Delaware Health Information Network (DHIN) has transitioned to a new Community Health Record: https://prodchr.dhin.net/account/login.

Please review the resources listed below for assistance logging in and using the new interface.

Items of note:

  • Administrative functionality is not yet available. If a user needs to be added or removed from your practice, please contact DHIN’s Service Desk or your Business Relationship Manager.
  • Multi-factor authentication is currently required upon each log in; post-deployment, this should be adjusted to once daily per user.
    • Some users experience an issue with receiving and employing MFA codes because their mobile devices aren’t automatically set to the correct date/time. Please follow these instructions to correct.
  • Searching by a common patient name may require an additional demographic identifier, such as date of birth, to call up the patient’s result.

The Community Health Record (CHR) is one of DHIN’s core services and it provides on demand access to 150M+ clinical results, Patient Demographics and Insurance Information for three million unique patients.

Instead of contacting individual facilities to obtain patient results, users of the Community Health Record have access to search for their patients and then view, print or download their results.

The Community Health Record includes data from all hospitals throughout Delaware and many of the stand-alone lab and radiology facilities.
