Enroll in DHIN: Save time, save money and provide better care.
It is the expressed expectation of DHIN for practices that make orders and receive medical results through DHIN to use DHIN as the exclusive source of delivery of these results (STAT results exempted from this requirement). Practices which utilize DHIN as a source of convenience (i.e. receiving medical results from sources in addition to DHIN, typically determined after about 3 months of being trained on DHIN) will be subject to an annual fee for this service. There is no charge for using DHIN’s core results delivery services for those practices that have signed-off on DHIN. Practices that use DHIN’s Community Health Record to search for information on their patients will be charged an annual fee.
By completing this form your practice will be on its way. Once we have received the form, your Relationship Manager will contact you to get your staff set up with DHIN. In order to complete enrollment process, the initial CHR Practice Training must be conducted on-site at the practice.
