Community Health Record

CHR icon

The Community Health Record (CHR) is one of DHIN’s core services and it provides on demand access to 150M+ clinical results, Patient Demographics and Insurance Information for three million unique patients.

Instead of contacting individual facilities to obtain patient results, users of the Community Health Record have access to search for their patients and then view, print or download their results.

The Community Health Record includes data from all hospitals throughout Delaware and many of the stand-alone lab and radiology facilities.

For more information about the Community Health Record, please contact your Business Relationship Manager or contact the DHIN Service Desk by filling out this form or email servicedesk@dhin.org or call toll-free (833) 625-6700 or (302) 480-1770.

Items of note:

  • ENS Prompt users can now view alerts for their patients directly in the CHR: Tutorial video
  • Administrative functionality is not yet available. If a user needs to be added or removed from your practice, please contact DHIN’s Service Desk or your Business Relationship Manager.
  • Searching by a common patient name may require an additional demographic identifier, such as date of birth, to call up the patient’s result.
  • Multi-factor authentication is currently required upon each log in.
    • Some users experience an issue with receiving and employing MFA codes because their mobile devices aren’t automatically set to the correct date/time. Please follow these instructions to correct.

If your practice or department would like a training session, please contact your Business Relationship Manager to schedule.

Testimonials

“DHIN makes it very easy for us when a patient calls and says that they’ve had lab work or they’ve had imaging done, that we’re able to print that out and have that in their chart and reviewed prior to their appointment. So, it helps us out a lot in terms of time and convenience.”
