DHIN and Availity Partner to Enhance Continuity of Care Document Usability 

Dover, Del. (Aug. 19, 2025) – Delaware Health Information Network (DHIN), the first state’s Health Information Exchange (HIE), has partnered with Availity, the nation’s largest real-time health information network, to improve the quality and consistency of Continuity of Care Documents (CCDs) shared across DHIN’s network. 

As part of the collaboration, DHIN will upload CCDs to Availity Clinical Data Solutions, an API-driven technology that ingests data from multiple sources and transforms it into a robust asset that can be effectively deployed into clinical and administrative workflows. The result is a clean, consistent and summarized CCD that is easier for providers to navigate and use at the point of care.
