Community Health Record

One of DHIN’s core solutions, the Community Health Record (CHR), provides on-demand access more than 150 million clinical results, patient demographics and insurance information for 5 million patients from 50 states.

Centralized Medical Information

Instead of contacting individual facilities to obtain patient results, DHIN customers can use the CHR to search for their patients and then view, print or download their results. The CHR includes data from all hospitals throughout Delaware as well as many stand-alone—and typically disconnected—lab and radiology facilities. This allows our customers to make informed decisions about their patients and provide them with the best possible care outcomes.

Event Notification System Coverage Area

The Event Notification System alerts you in real time when one of your patients has an encounter at any hospital in Delaware or DHIN-participating hospital across the region.

Coverage Area

DHIN currently receives alerts in real time from any patient admission, intrafacility transfer or discharge in any Delaware hospital as well as for Delaware residents treated in:

  • Delaware
  • Maryland
  • Pennsylvania
  • Washington, D.C.
  • West Virginia

Who Can Get Notifications?

The Event Notification System alerts DHIN users who need to track certain patient populations. These alerts may include the hospital event type (e.g., admission, discharge, transfer) as well as updated patient demographics and recent prior admissions.

Sign Up for the Event Notification System

To sign up for the Event Notification System, you will need to input three pieces of information:

  • 1. A patient list that you would like to track.
  • 2. Trigger events for which you would like to get alerts (ED admissions, discharges, etc.).
  • 3. How you want to receive alerts (secure FTP, via online secure portal, direct feed, etc.).

Care Summary Exchange

With care summaries contributed by practices and providers across the state, there is a good chance you have a patient with a care summary in the Community Health Record (CHR). Care summaries can be accessed under the Clinical Documents Tasks card inside a patient’s CHR chart.
