Submit a Freedom of Information Act (FOIA) Request
To submit a Freedom of Information Act (FOIA) request, as outlined in Delaware’s Freedom of Information Act, please use the form below. Requests for personal health information are not covered by FOIA. Please follow the instructions above for those requests.
For FOIA requests, we might need more information regarding your request and will contact you accordingly. Some requests may take up to 10 business days to complete. Please note, requests for copies greater than 20 pages will require a fee of 10 cents per page.
