DHIN and Availity Partner to Enhance Continuity of Care Document Usability
Dover, Del. (Aug. 19, 2025) – Delaware Health Information Network (DHIN), the first state’s Health Information Exchange (HIE), has partnered with Availity, the nation’s largest real-time health information network, to improve the quality and consistency of Continuity of Care Documents (CCDs) shared across DHIN’s network.
As part of the collaboration, DHIN will upload CCDs to Availity Clinical Data Solutions, an API-driven technology that ingests data from multiple sources and transforms it into a robust asset that can be effectively deployed into clinical and administrative workflows. The result is a clean, consistent and summarized CCD that is easier for providers to navigate and use at the point of care.
Since going live in December 2024, DHIN has already processed over 340,000 CCDs, covering over 125,000 patients through the Availity platform, enhancing the quality and shareability of valuable clinical information.

It’s been a great working relationship with Availity to get this pipeline established and seeing this data improved and sharing with a larger group has been great for DHIN.”
A CCD is a structured electronic summary of a patient’s health record, designed to support care transitions between providers. By improving communication, standardizing clinical data exchange and reducing medical errors, CCDs play a vital role in delivering coordinated, high-quality care.
However, CCDs often vary in structure due to inconsistent adherence to standards and differences across electronic medical record (EMR) systems. This variability makes it difficult for providers to access relevant data quickly and efficiently. The partnership between DHIN and Availity addresses these challenges by transforming CCDs into standardized, usable formats.

The collaboration will continue to scale, with both organizations working to increase CCD volume, extract discrete data elements, and further integrate the improved documents into DHIN’s infrastructure. The goal: improve clinical workflow, accelerate transactions, and reduce delays in patient care.
